The application form may be downloaded as a Word document or Word document for JAWS (requires Microsoft Word on your computer). Left click once or press the Enter key on the link of your choice. The application form will open up in your browser.

At the top of your browser menu, click on File. From the drop down menu, select Save As. You may also press F12 to bring up the Save As dialog box. (If you are downloading the pdf version, you may not have the option to 'Save As.' Instead, you will need to locate the disk icon at the upper left hand corner of the document window and select it). A Windows menu will pop up, allowing you to select where you want to save the file on your hard drive. Once you have selected the desired folder or directory, press the Enter button on your keyboard, or click the Save button at the lower left hand corner of the pop up menu.

The application is now saved on your hard drive. You may double click on its icon or select it and press Enter to open it up.

You may type information directly into Word formatted application forms (we recommend you save a copy of the original downloaded form prior to typing in it, in case you make a mistake). Information should be typed or neatly hand printed in dark blue or black ink.
